Medical Insurance: You are eligible for Medical coverage on the first day of the month following 90 days of employment.

Life and Accidental Death & Dismemberment (AD&D) Insurance: The Company provides employees who enroll in the Group Medical Plan $15,000 Personal Life Insurance and $15,000 AD&D Insurance. Spouses who are enrolled in the Medical Plan are provided $5,000 Dependent Life Insurance, and children ages 6 months to 19 years old who are enrolled in the Medical Plan are provided $1,000 Dependent Life Insurance.

Optional Dental: You are eligible for Optional Dental coverage on the first day of the month following 30 days of employment.

Group Short-Term Disability Insurance: The Company provides you with short-term disability insurance after 90 days.

Optional Long-Term Disability Insurance: You are able to purchase Optional Long-Term Disability (LTD) Insurance. You are eligible for coverage 6 months after you enroll in the LTD Plan.

Optional Life Insurance: You can purchase up to 5 times your annual salary in affordable Life Insurance. You may purchase up to $100,000 on a guaranteed basis, while amounts greater than $100,000 may be purchased after passing a medical questionnaire. You can purchase Spouse Coverage for half of what you are covered, and you can purchase Dependent Child Coverage from $2,500 up to $10,000.

401k Retirement Savings Plan: You can start saving in the 401k Retirement Savings Plan on the first of the next quarter following six months of employment. Empire matches $.50 for each $1.00 you save, up to 6% of your annual taxable wages. The matching contributions become yours in 20% steps over a five-year vesting period.

Vacation Pay: After the first year of employment, Empire pays 1 week of vacation pay and after three years, Empire pays 2 weeks of vacation pay.

Driver Referral Bonus: $250. at sign on, plus $250. after 90 days of service.

Rider Program: Spouses may ride with you as early as the first dispatch and may ride with you for as long as you like. After 30 days, other family members over 12 years of age may ride with you upon request. The Company pays for the Passenger Insurance.

Pets Policy: You may have your small cat or dog accompany you in the truck. You must deposit $500 in an escrow account for any future cleaning or repairs that would be required to restore the cab to a clean, fresh, and original condition. The $500 escrow is returned to you when you vacate the truck and the cab does not require extensive cleaning or repair.
